This page contains downloads for the Cancer Outcomes and Services Dataset (COSD) Version 7.0.

Is this the correct version of COSD to use?

  • Version 7.0 must be used for all submissions from 1 July 2017 diagnoses onwards. 
  • Either v7.0 or v8.0 can be used for diagnoses between 1 Apr 2018 and 30 Jun 2018 
  • Version 8.0 only must be used for all submissions from 1 July 2018 diagnoses onwards. Any exceptions to this must be agreed with the National Cancer Registration and Analysis Service (NCRAS)

Pathology dataset

Following consultation with Laboratory Information Management System (LIMS) Suppliers and the Chair of the Royal College of Pathologists (Working Group on Cancer Services), minor alterations to the structure or contents of the dataset have been made in the updated versions of the pathology documents below. Details of the changes are in the spreadsheet summary.

This dataset and schema are now frozen and final until the next iteration of COSD is released.

Schema

To download either the Main COSD or Pathology schema packs, you first need to create a TRUD account.

How to create a TRUD account

See the former HSCIC (now NHS Digital) XML Schema TRUD Download webpage for further information and to be guided through the process or

Go straight to the HSCIC TRUD website:

      • Select 'create one now' (directly under 'log in')
      • Fill out the online form
      • You will be sent a password
      • Log in with your new password. Once logged in you can change your password via 'Manage account' (top right hand of the screen)

How to download a schema pack

Once you have a TRUD account:

      • Go to the HSCIC TRUD website and log in
      • Select 'DD XML Schemas' from the 'Downloads' section (far left of the screen)
      • Identify the schema pack you want and 'subscribe to this dataset'
      • Tick 'I accept the licence' and then 'Request this subscription'
      • You will receive a confirmation email
      • Download the required schema pack

Conformance

A revised Conformance Framework has been published to align with v7.0. This current Framework v6.1a, can be downloaded from the main dataset documentation above.

It has been amended to take account of the changes in v7.0 and will be reviewed at the end of 2017.

Why change to COSD v7.0?

It is important to constantly maintain clinical accuracy and engagement with the MDT teams. We needed to review and make some important changes to the dataset. Many of the dataset items were agreed in 2012 and had become outdated. There was also a lot of duplication across the dataset. 

The COSD v7.0 amendments were made after consultation with software suppliers, specialists with Public Health England and NHS England, as well as extensive consultation with the Site Specific Clinical Reference Groups (SSCRGs). In total, 94 data items were removed and 82 new ones added.

Work is now underway to complete the review of COSD in v8.0.  The SSCRGs have been asked to review all their data over a 6 month period. With this review we will be looking to:

      • Re-allocate CTYA within its tumour site specific parent groups (but remaining its own identity as CTYA)
      • Create a new reporting pathway to improve the accurate recording of recurrence, secondary and metastatic disease
      • Conduct a total review of all pathology data items to ensure they still meet the clinical accuracy demanded within the Royal College of Pathologists own minimum datasets
      • Add a date when the trial started (for clinical trials)
      • Work with CWT and Clinical Audits, to ensure all datasets are better aligned and to try to remove as much duplication and burden of data collection as possible.

CancerStats has also changed this year. The following reported areas have been added:

      • Incidence, Survival and Mortality
        • COSD
          • Level 1, 2 and 3
          • Updated staging sheets
          • Submission dates
          • Document library
      • National Lung Cancer Audit (NLCA)
      • National Prostate Cancer Audit (NPCA)
      • Clinical Headline Indicators (CHI)
      • New areas to come on Pathology conformance reporting
